Already a card holder? Please remember to carry your card with you at all times. If not, you’ll need to pay any medical expenses directly and then submit a claim to be reimbursed (including any receipts and medical documents). Note that we only accept claims for treatment from hospitals and clinics within our network.

FWD Classy Care is our prestige inpatient and outpatient offering with a range of added customer extras that you can access if you’re in hospital. Check your FWD insurance card to find out if your plan is eligible and then simply select “FWD Classy Care” from the dropdown on the map.

Employee Benefits customer? If you need treatment at a hospital or clinic in our network, you can either show your FWD Insurance card or simply give your name and details.
